Calumet County Public Health Telephone Survey

Calumet County Public health is partnering with other local health departments and health systems to conduct collaborative community health telephone survey.

– If you get a call from Management Decisions, Calumet County Public Health urge you to pick up the phone and participate. Starting in December, a community health telephone survey will go out to residents of Calumet, Outagamie and Winnebago counties to evaluate the community’s most pressing health needs. The survey, a collaborative initiative among the five public health departments and four local health systems, aims to interview a random sampling of about 1,500 households.

The anonymous survey focuses on issues such as access to health care, tobacco and alcohol use, diet, physical activity, cancer prevention, heart health, injury prevention, children’s health, mental health, and chronic disease.

According to Menasha Public Health Director Nancy McKenney, the new data will provide current information and help identify trends over time. “This is the fourth time the public health organizations and health systems have come together to conduct this research. Under the Affordable Care Act, non-profit health systems are required to conduct health needs assessments of the communities they serve every three years. Public health departments are also required to complete community health assessments,” she noted.

“Working together to collect data provides cost and time efficiencies. Plus, it enables synergistic conversations and planning to address the most pressing health needs identified by our residents,” said Paula Morgen, Director of Community Health at Theda Care. Results will be released in later 2018.

The community health survey is aimed at meeting several objectives:

  • Documenting local health status related to Wisconsin’s State Health Plan priorities
  • Identifying local behaviors or risk factors that present opportunities for health promotion and health care cost reduction
  • Tracking the success of community health improvement activities
  • Allowing partners to document population health indicators to guide health improvement efforts

Community Health Survey Local Health Partners

The Fox Valley Community Health Improvement Coalition is coordinating the survey. It is being conducted by Management Decisions in partnership with JKV Research, an independent research firm specializing in community surveys. Fox Valley Community Health Improvement Coalition members include:
